CoolSculpting is a very popular procedure in my office! It works by freezing the fat cells beneath your skin while keeping the surrounding skin, muscle, and nerve cells healthy and intact – a process called “selective cryolipolysis.”

This radical (and very cool) concept was initially explored by scientists who observed an interesting phenomenon called “popsicle panniculitis” in kids. Scientists noticed that some children who sucked on lots of ice popsicles actually developed dimpling in their cheeks as a result of permanent death of fat cells exposed to cold temperatures.

Obviously this doesn’t happen in every child who sucks on a popsicle (thank goodness), but there was some perfect combination of just the right temperature, just the right amount of pressure, and just the right duration of time that led to this fat reduction.

This observation led scientists to question whether they could develop a very controlled means of freezing certain unwanted bulges of fat for the purposes to body contouring. Thus was born the technology surrounding CoolSculpting.

It is completely non-invasive, which means no needles, no cutting, no scarring and very little recovery time.

Until recently, treating cellulite with a long term impact has been an enormous challenge. Most options were simply quick fixes which worked for a few hours to, at best, a few days.

Now, we have an FDA-approved medical procedure called Cellfina™ which targets the root of the problem, revealing smooth, beautiful skin. This procedure has an excellent patient satisfaction record and the results last for three years, at minimum, and likely a lot longer (based on clinical trial results)! What causes cellulite?

First, to explain how Cellfina™ works, I have to explain what causes cellulite. We all have fat deposits underneath our skin. We also have fibrous connective tissue or bands (think of rubber bands) which tether our skin to our underlying tissue. Cellulite, or dimpling, occurs when the underlying fat deposits begin to push through weak spots in the connective tissue under the skin, but the bands keep pulling in certain spots—thus those dreaded “dimples” we all know too well! How does Cellfina™ work?

The Cellfina™ procedure works by using a small motorized needle to break those weak bands of connective tissue and release them, which in turn, allows the skin to snap back and smooth itself out.

This procedure is ideal for skin dimpling on the buttocks or upper backs of the thighs. I find that it is not as effective for the front of the thighs or the area above the knees.

Is it Safe?

The side effects reported from this procedure are all mild to moderate and include bruising, soreness, and some swelling. These side effects last for a few days to approximately two weeks.

Your Consultation:

You will first come into my office for a consult to determine whether you are a good candidate for Cellfina™. We will take photographs of the area we plan to treat. We mark out your cellulite dimples with a sharpie pen (not the most fun, but just imagine them all disappearing after the treatment).

If I determine that you are a good candidate for this procedure, I will advise you that, two weeks before your procedure, you must stop taking Aleve, Fish Oil, Advil and Ibuprofen.

Your Cellfina™ Procedure:

The entire procedure takes about an hour. I begin by administering local anesthesia and you will feel a few pinches and a bit of stinging. Then, the area we will treat becomes numb and you have no discomfort. I offer my patients a small dose of Valium before we begin in order to help them relax and to take the edge off. I recommend bringing headphones because, as you can hear from this video, there is a loud sound from the device I use.

After Your Procedure:

When we are done, I will wrap the area I treated in sterile gauze and advise you to take Tylenol for discomfort. You will put a compression garment on top of the bandages – this garment can be tight fitting bike shorts, spanx, Shapewear, or tight fitting leggings. You will be somewhat sore and bruised in the treated area especially those first two days after the treatment. I have a patient who went back to work the next day, but I generally recommend to take one day off. After one day, you will be up and about doing light activities. I recommend taking Tylenol.

You should continue to wear your compression garments for two weeks after the procedure. This will minimize swelling and bruising.

You will see REAL and EXCELLENT results within three days of the procedure and the results will continue to improve for up to one year after the procedure!

I love attending and presenting at the American Academy of Dermatology’s Beauty Breakthroughs Events in NYC because it is like a think tank for cutting edge information in my field. One of the hot topics presented at this year’s event by my esteemed colleague and friend, Dr. Chris Adigun, was the safety of gel manicures, especially in view of the LED and UV lamps used to cure gel polish.

I wanted to share some of the latest information available on this topic because it is something so many of us do without giving it a second thought!

We know why we all love gel polish. It lasts up to three weeks without a single chip and is more glossy and shiny than any other type of polish I’ve seen. But, there is always the question of safety when you place your hands under the lamp to cure the polish. Your salon either uses a UV lamp (if you wait for two minutes for the polish to cure, this is a UV lamp) or an LED lamp (if you wait for between 5 and 45 seconds for your polish to cure, this is an LED lamp) as a necessary step in curing gel polish. BOTH UV and LED lamps emit UV radiation. Many people think the LED lamp is safer, but in reality, it is just a shorter, intense periods of exposure to UVA rays, which are linked to skin aging and skin cancer.

But don’t despair. There are some safety precautions that you can take before placing your hands under the lamps. Either apply UVA/UVB sunscreen to your hands 20 minutes prior to placing them under the lamps or use anti-UV gloves, which are available online or possibly at your salon.

Another option which doesn’t require a UV or LED Lamp is Sally Hansen Miracle Gel, which is a part lacquer and part gel polish. The Color Coat and Top Coat formulas both contain an oligomer (a molecule), and the Top Coat also contains a photoinitiator (a chemical compound). The photoinitiator activates a bond between the Color and Top Coat, thus curing the formula in natural light without the need of an LED/UV lamp. I’ve tried it and it looks like a gel manicure from the salon and lasted about the same amount of time. Always good to have options!

As promised in my post on Kybella, here are some other extremely effective options to address the dreaded double chin (or “turkey neck”):

The New CoolSculpting COOLMINITM: I have already blogged about the incredibly cool, groundbreaking and FDA approved technology behind the CoolSculpting procedure.

Here, I want to focus on the new applicator, which is called the COOLMINI. Whereas CoolSculpting is generally used in the thigh, abdomen and flank or “love handles”, this newly shaped and designed applicator is specifically designed to treat smaller pockets of fat, including the chin area.

Your Virtual COOLMINI Appointment:

First, I apply a cool gel to the skin underneath your chin and position the applicator. You will then feel a gentle suction.  You are lying down on your back the whole time – and the procedure is painless – so feel free to bring a sleep mask to cover your eyes if you want to black out any light and take a snooze. Here is a video of the procedure in my office:

The procedure takes about one hour. After the hour is up, I gently massage the skin for 2 minutes and you might feel a slight ache while the nerves in the skin warm up and regain their sensation. Most people say it feels like pins and needles for a few minutes.

After the procedure, your skin may be a little bit pink for an hour or so, but there is NO swelling and NO bruising. You might have a bit of temporary numbness for a few days afterwards in the area we treated.

Patients may see results from a COOLMINI treatment in as early as three weeks, but the most dramatic results are seen 1-3 months following treatment. Like Kybella, this is a PERMANENT SOLUTION.

A key consideration before you have this procedure is that your neck has to fit into the COOLMINI applicator. It is generally the best fit for a wider neck. Therefore, I recommend that you see a doctor who can perform both procedures so that if you don’t fit properly into the COOLMINI applicator, your doctor can offer Kybella as an alternate option.

You typically need 1 to 2 treatments (each for one hour, as I mentioned) which should be spaced approximately 6 weeks apart.

Price Point: Approximately $1000/treatment.

Nectifirm Cream: If you want to treat your turkey neck at home, this cream by Revision Skincare is surprisingly effective! I recommend applying the cream to your neck and chin area consistently at night after cleansing. The skin underneath your chin will become significantly firmer and more lifted, you will notice a reduction in lines and wrinkles, and the skin “creping” will be smoothed. My patients have given very positive reviews of this cream. At a relatively modest price point, this is a promising option, especially given that it includes high quality ingredients including Tripeptide, which I believe is a key factor in driving long term results. Price point: Approximately $50.

SkinCeuticals Skin Firming Cream: Another patient favorite from a brand which is known as a driving force for scientific breakthroughs! This light feeling cream is excellent for smoothing, firming and toning your skin, including the skin underneath your chin. It addresses fine lines and wrinkles and leaves your skin with an overall refreshed appearance and lift. My patients have repeatedly told me that the slightly higher price point is well worth it for the results! Price Point: Approximately $110.

I Love My Turkey Neck [Said No One Ever]!

As Thanksgiving quickly approaches, I wanted to dedicate this blog post to the turkey neck (or double chin) and how I can help make it go away forever, as I discussed on Good Morning America.

The turkey neck, or double chin (which is medically referred to as “submental fullness” or “submental fat”) is an equal opportunity issue. It presents in men, women, thin people, and heavier individuals alike and I have yet to meet a patient who would like to keep it. In fact, even apart from the undesirable aesthetic of a turkey neck, many of my male patients have sought out this procedure due to the daily discomfort and feeling of suffocation created by having submental fullness and having to wear a shirt and tie every day for work.

Thankfully, the FDA has approved Kybella (deoxycholic acid) as a permanent treatment for the turkey neck. Deoxycholic acid is naturally produced by our bodies and helps to absorb fats. Kybella is a drug which is identical to this naturally occurring acid. When injected into our “submental fat”, Kybella physically destroys and permanently dissolves the fat cells which create the double chin.

 

Your Virtual Kybella Appointment:

Before we begin the Kybella injection treatment, I clean and numb the area underneath your chin.

I then apply a treatment “grid”, which is like a temporary tattoo, to act as an injection guide. I inject Kybella next to each “dot” on the grid. You will feel a slight prick and a burning sensation as the medication is melting the fat away.

The entire procedure takes approximately 15 minutes.

The most significant side effect of this procedure is the substantial swelling which occurs afterwards and can last up to two weeks. However, even with this side effect, my Kybella patients all agree that the procedure was well worth it and they would do it again without hesitation. I tell my patients that winter is the perfect time to have Kybella because you can camouflage the swelling by wearing turtlenecks or scarves.

Other common side effects of Kybella include some bruising, redness, and sensitivity or numbness in the treatment area, all of which are temporary.

Typically, to achieve my patients’ desired results, we need 2 to 4 treatments approximately 6 to 8 weeks apart. This aspect of the procedure is customized.

And finally, the result is PERMANENT!! Your turkey neck is gone forever and I promise you won’t miss it!

Generally speaking, the ideal patient for CoolSculpting is someone who is close to their ideal body weight, but has one or more stubborn bulges of fat that just won’t disappear with a healthy diet and regular exercise.  For example, if you just can’t get rid of that extra bulge right under your belly button, that muffin top, or generous love handles, CoolSculpting is perfect for that scenario.

However, I have also recently started to use CoolSculpting on patients who have a substantial amount of weight to lose – upwards of 20 pounds – in order to “jump start” their weight loss plan. My patients tell me that they are extremely motivated by seeing tangible results and feel empowered and ready to work toward losing the rest. My patients and I have been very excited that this strategy is proving to be so effective!

Your Virtual CoolSculpting Appointment

What to Expect: I apply a CoolSculpting handpiece to your skin overlying the “bulge” or “trouble spot” we want to slim down. Ideal areas to treat include love handles, rolls in the belly region, or unwanted fat on your back. Next, I turn on the suction which “sucks” your bulge into the hand-piece. This part of the procedure can be somewhat startling especially for first-timers and for patients for whom we select the largest of all the handpieces (called the 8.0 handpiece). In those cases, patients often use some variation of the phrase “it took my breath away for an instant” to describe the initial moment when the suction is applied. Once you are positioned comfortably in a place you can envision yourself relaxing for the next hour, I turn on the cooling part of the device. The first 10 minutes can be moderately uncomfortable –ranging from a pulling sensation to a slight cramping sensation. The larger the handpiece we use, the more uncomfortable those first 10 minutes can be, but the flip side is that the larger handpieces also remove more fat. I call them “debulking” treatments, because they produce that desired “wow” effect when you button your favorite jeans two months later. After those 10 minutes, the nerves have become numb from the cold, and that’s when most of my patients do work on their blackberries, watch a show on their Ipads, read magazines, or simply fall asleep! My patients describe this part of the procedure, which is the majority of the procedure, as comfortable and completely painless.

When the session is over, I remove the hand-piece and massage the area that I can best describe as having a slushy-like consistency. Once this fat defrosts (generally within five minutes), you can return immediately to your normal activities. One patient I treated went straight to a boot camp class and another went directly to Soul Cycle! Although the downtime is minimal for most, you can experience some moderate bruising in the area treated, and many people feel a bit of numbness or tingling on the skin overlying the fat that was treated for several days or even a few weeks.

Over the next few months, your body slowly and naturally disposes of the damaged fat cells without any spike of cholesterol or unhealthy fats in the bloodstream — this was carefully studied before the device was FDA approved. In patients who maintain a stable diet and exercise program (no crazy binges or weight fluctuations), the results are very long-lasting.

Time-Frame: A session can involve as little as one hour, but most of my patients opt to do between 4 and 6 hours total of CoolSculpting. What does this mean? Each time I place the handpiece on a part of your body and let the fat “freeze” it takes one hour. For each hour of fat freezing, about 22% of your fat cells in that area die. Now, let’s do a demonstration I do with my patients. Grab the bulge you hate the most on your body (for most, it’s the bulge right under your belly button or love handles). Imagine almost a quarter of that fat gone (22%) after a one hour long treatment. Not bad, but now imagine doubling that result by returning for a second hour of treatment in the same exact spot. Go on, grab that fat bulge one more time and now envision almost HALF of it gone. Now those are the results most people are looking for.

Additionally, something exciting is on the horizon! CoolSculpting is in the process of being FDA approved for use on double chins and “turkey necks”. Stay tuned!!

I was game when my friend Carolyn (an instructor at CoreLab) suggested the Lagree Fitness Workout which is performed on a Megaformer.

This workout is a modified version of Pilates which fatigues your muscles, but is still low impact. It helps to build your core and has a West Coast vibe.

I’ve been loving my body’s response. I used to gravitate toward high intensity workouts like extreme boot camp and kick boxing. When I started mixing in lower impact workouts like the Megaformer, I wasn’t dripping with sweat and totally worn out at the end of each workout, but I started seeing more muscle definition and I FELT stronger and more focused throughout the day.

Now, I alternate this kind of low impact workout with my higher intensity cardio workouts (spinning/running) and aim for about two Megaformer or Pilates-type workouts per week.

Whether you want lush, festive red lips for your holiday party or a sexy, berry-hued pout for a kiss under the mistletoe, plump, full lips are all the rage right now!

As I discussed on Good Morning America, I achieve the perfect pucker for my patients with a few quick injections of temporary lip fillers such as Restylane Silk or Juvederm Ultra XC. These fillers contain hyaluronic acid, which is a sugar gel naturally found in the body. They work by attracting water into the lips, hydrating the lips from beneath the surface to create that beautiful fullness. The result lasts for an impressive 6 months to a year!

From an aesthetic standpoint, I want my patients to look natural, not “done.” Over-filled, exaggerated lips are not beautiful, in my opinion. Therefore, to keep it real, as I like to say to my patients, I respect the “Golden Ratio”, which is a precise, mathematical ratio of proportions which our brains read as beautiful. This ratio is used in art, in the Egyptian Pyramids (it was considered sacred by ancient Egyptians), and is a goal of cosmetic dermatologists. The closer your practitioner is able to get to this ratio, the more naturally stunning your results will be!

If you aren’t ready to commit to injections or just want a quick fix for a night out, there are also great options on the market! These are some of my favorite lip plumping products:

Manuka Doctor ApiRefine Lip Enhancer: This plumper uses purified bee venom, which is often referred to as “Nature’s Botox” and Manuka honey. The purified bee venom encourages collagen growth for long term plumping! I found this product to be non-irritating and had great results! $29.99 at CVS stores or online at www.manukadoctor.com.

FusionBeauty XL Advanced Lip Plumping Therapy: This lip plumper uses a patented dehydrated Marine Collagen alongside tried and true hyaluronic acid to deliver a moisturized, full pout. I felt tingling, but no discomfort and loved the results. $50.00 at fusionbeauty.com (Also available in XXL Advanced Lip Plumping Therapy Intense for $57).

And, for the most cost effective option, there are natural, DIY alternatives which you can easily make at home!

First, Cinnamon works by irritating the lips, which makes them swell (so, you will want to save this option for special occasions, not every day use!). Here is my favorite DIY Cinnamon lip plumper recipe:

  • 1 teaspoon of beeswax
  • 1 teaspoon of cinnamon
  • 1/2 teaspoon of vanilla extract.
  • Leave your plumper on for about 10 minutes and rinse off with warm water. (You should feel a tingling sensation. If you feel any sort of pain or burning, remove immediately and follow with Aquaphor or Petroleum Jelly).

You can also add a few drops of cinnamon essential oil, peppermint essential oil, or cayenne pepper oil to your regular lip balm or to a little bit of petroleum jelly and use as a lip balm to stimulate blood flow to the lips and to create a slight irritation which will enhance and plump your lips temporarily!
